Degloving injury occurred to 21 years old male while coming from religious pilgrimage . Patient had total degloving of forearm skin , with extensive crushing of muscles. Patient had small liver injury as well as degloving of right thigh as well.   Pt was investigated and planne for debridement. After debridement patient was taken up for flap cover.    ALT myocutaneous was harvested and transferred ,after inset flap was vessels were connected ,artery to radial artery end to side. Vein was connected to superficial vein.    Post operative day patient was taken for haematoma evacuation, and after one week patient was discharged. Surgery was operated at Metro hospital Faridabad
 Patient was admitted in a orthopedic hospital in Palwal his home town.

Orthopedic surgeon Dr Sunil Goel played a great role in finishing up rest of work. Patient  required regular dressing , posterior thigh wound and flap donor site was skin grafted. Blood transfusion was given and appropriate other supportive care was given.After 2 months his most of injury healed and patient was ready to go back to his old life. 


Immediate debridement and wound cover is important in early recovery from injuries . It prevents desiccation of  tendons ,preserve muscle and neurovascular structure.Microvascular surgery is better than pedicled flaps as it allows early mobilization , less downtime and additional surgery.

Patient was a 75 years old lady from Iraq who under went joint replacement surgery in India. She developed post operative wound dehiscence which resulted in exposed implant.For two months orthopedic surgeon and plastic surgeon for some reason or another kept the wound open did took no decision to close it .Patient under went one debridement and VAC therapy. Then I received a call from interpreter and I saw the patient.We decided that few days of VAC therapy and in consultation with ortho surgeon we decided date of surgery. We operated and after debridement I did a medial gatrocnemius flap cover and skin grafting. Post operative patient had fever for two days and dressing was soaked. We had placed a drain which did not worked ,so we removed drain and started daily dressing and betadine, hydrogen peroxide irrigation of the wound.
